Background & aim: Recently, the incidence of adnexal masses during pregnancy has increased, possibly due to delayed gestational age and advances in imaging or more frequent use of ultrasound. As a side effect, ovarian torsion can occur in less than 1% of pregnancies and is more common on the right side. The aim of the present study was to present a rare case of left ovarian torsion due to a large adnexal mass in a woman at 16 weeks of gestation. Case report: A 32-year-old woman in her third pregnancy at 16 weeks of gestation presented to the obstetric emergency department of Imam Sajjad Hospital if Yasuj, Iran, with severe intermittent lower left abdominal pain. Ultrasonography revealed a large left ovarian cyst measuring 15 × 12 cm. Due to worsening pain and lack of response to conservative management, the patient underwent laparotomy. The ovary was found to be twisted three times; detorsion was performed, and the cyst was excised. The postoperative course was uneventful, and the pregnancy continued to term without complications. Conclusion: Ovarian torsion is rare in pregnancy, but in the presence of large adnexal masses, it must be diagnosed and treated promptly to preserve ovarian function and ensure a safe pregnancy.
